With Govstack Forms, you can view audit reports, form reports and financial reports for each form you create. You can also export each report as a CSV (Comma Separated Values) file. This allows you to easily transfer the information into a database or Excel file.
To access form reports:
- Navigate to the appropriate form or folder in Govstack Forms
- Click the “Generate Reports” button located next to the form. If you're already in the form, click the "Reports" tab from the toolbar at the top of the form
- Choose the report you want to view
Audit reports
The Audit Summary Report shows all changes that have been made to a form. You can filter the report by date by using the “Event Date” field. Set a date-range for the report.
An Audit Summary Report will provide the following information about changes to your form:
- Date
- User name
- Action
- Form name
Form reports
Form reports provide a detailed report on all responses received for a form. You can filter the report by date, keyword or by the status of the response under the "Only Show" dropdown. For example, you could include all completed responses, partially completed responses or all responses regardless of the completion status. Under the "Format" dropdown, you can choose to export your form report in a compact view or expanded view. To only view specific responses to specific questions in your report, check the box next to "Filter responses based on answers to questions" and select the questions and responses you want to include in the report.

- Completion date
- IP address
- Browser information
- Reference number
